LEE MEMORIAL HEALTH SYSTEM FOUNDATION holds many events to support the Trauma Program which provides life saving emergency services for our seven county region including Lee, Charlotte, Collier, Hendry, Glades, Desoto and Sarasota counties.  An upcoming event being held July 25, 2009 is the 7th Annual David Lee Root, Jr., Memorial Fishing Tournament.  We would appreciate your consideration of a cash donation for the tournament.  

 

Platinum Sponsor:  $2500 (entry to Tournament, 2 anglers; company name and logo on T-shirt; & your company banner hanging during tournament.

Gold Sponsor:  $1000 (entry to Tournament, 2 anglers; company name and logo on T-Shirt; & your company banner hanging during Tournament.)

Hat Sponsor:  $1000  (maximum of 2 sponsors)

T-Shirt Sponsor:  $250 (company name and logo on tee shirt).

 

All sponsorship contributions must be received no later than June 15th, 2009.  Make checks payable to David Lee Root Jr., Memorial Fund.
